GROUNDWRK Fitness Liability Waiver & Release
1. Assumption of Risk
I understand that fitness activities involve risks including injury, accidents, or medical events. I voluntarily choose to participate and accept full responsibility for my health and safety.
2. Medical Clearance
I confirm that I am physically capable of participating and have disclosed any relevant medical conditions to my instructor.
3. Waiver & Release
I release and discharge GROUNDWRK, its owners, instructors, and staff from all liability, claims, or demands related to participation in fitness activities, whether caused by negligence or otherwise.
4. Emergency Care
In the event of an emergency, I authorize staff to seek medical care on my behalf and accept financial responsibility for related costs.
